How to knit corner to corner and end up with a square

When starting with one stitch and increase 1 beginning every row and when right length then decrease one stitch every row why does my knitting not make a square but end up with too many rows on the decreasing.

I think it's possible that you skipped a few decreases that's why you have to make up extra rows for those stitches. This make the decrease side longer. I can't think of other reason, can you?

I find that the last row of my square knits always pointy. I like to just work the last 3 sts together.
